Shining Star Award Nomination Form

Shining Star Awards highlight a member that has demonstrated exemplary actions for the chapter.

Criteria:

Nominators must be a member of MPIPHL of at least one year.

Nominees must be a member of MPIPHL and a volunteer on one of the chapter's committees.  Board members are eligible for Shining Star Nominations.
